1.UDID CARD Application process

UDID portal : https://www.swavlambancard.gov.in/

  1. Registration:
    Visit the UDID portal and register as a Person with Disability (PwD).
  1. Online Application:
    Log in to the portal using your credentials and apply online for a disability certificate.
  1. Document Upload:
    Upload necessary documents, including a color passport photo, income proof, identity proof, and caste certificate (if applicable).
  1. Verification:
    The application will be verified by the CMO office or relevant medical authority.
  1. Assessment:
    The medical authority will refer the applicant to a specialist or medical board for assessment of the disability.
  1. Certificate and Card Issuance:
    Once the assessment is complete, the disability certificate and UDID card will be generated and dispatched to the applicant.

2. MEDICAL BOARD APPLICATION PROCESS

How KSSM issues Disability Certificates:
KSSM conducts Disability Certification Camps in collaboration with local self-governing bodies, Social Justice Department officials, and Health officials. These camps aim to identify and certify individuals with disabilities, ensuring they receive the necessary support.

How to obtain a Disability Certificate:

  1. Application:
    Individuals can apply for a Disability Certificate by submitting an application form, usually to the Anganwadi teacher/worker.
  1. Camp Notification:
    The date and venue of the camp will be communicated to the applicant.
  1. Assessment:
    Medical professionals at the camp will assess the applicant’s disability and issue the certificate if eligible.
  1. Permanent ID Card Holders:
    Individuals already possessing a permanent disability ID card may not need to attend the camp and can apply directly to the Executive Director of KSSM with the necessary documents.
